FSU ATHLETIC TRAINING MEDICAL CLEARANCE PROCESS

Listed below are the various procedures and forms required for the medical clearance process for both new/incoming and returning student-athletes to FSU. All student-athletes must be medically cleared prior to beginning workouts, practices or receiving any athletic equipment.

The many forms required are to comply with both NCAA and FSU rules and regulations. Please do not fax or email the required forms as some information cannot be read when faxed.  All forms must be returned by mail to the address listed below prior to August 1.

Please follow the appropriate column path below based on whether you are a returning student-athlete or a new/incoming student-athlete or transfer.  All files are available both as a PDF or Microsoft Word document.
